  • Workman’s Comp Evaluations
  • Bariatric Evaluations
  • Emotional Support Animal (ESA) Evaluations
  • Forensic Evaluations
  • Legal Evaluations
  • Fitness for Duty Evaluations
  • Permanent Disability Evaluations

Records are released only to the requesting entity, not directly to the patient. Physician-completed forms are finished within 5 business days, and the provider hourly rate applies.
