Office Policies
These policies help patients understand how our office operates and what to expect before, during, and after treatment.
Appointments
- Your appointment time is reserved only for you. Appointments must be confirmed within twenty-four business hours of the scheduled time or they may be canceled.
- If you need to cancel, you must do so with at least twenty-four business hours’ notice by calling or emailing the office.
- Late cancellation fees are $100 for follow-up appointments and $200 for new patient appointments. Late cancellations or no-show fees are automatically applied to the card on file.
Emergency Appointments
- If you have a life-threatening emergency, call 911 or go to the nearest emergency room.
- Our office is not equipped to manage psychiatric or medical emergencies on site.
- For urgent matters that cannot wait until the next business day, email contact@mybiomental.com to request a callback.
Exclusion Criteria and Things to Consider
- We do not provide legal evaluations and may decline disability paperwork at the physician’s discretion.
- If you have several systemic illnesses or are above the age threshold our office advises, we may recommend a psychosomatic or geriatric psychiatry specialist.
- Patients taking multiple controlled substances and unwilling to consider other options should seek a more accommodating office.
- We do not accept Buprenorphine candidates who plan to take tranquilizers such as Xanax, Klonopin, or Ambien together with treatment.
- We do not accept Buprenorphine candidates who are planning to obtain or currently hold a medical marijuana card.
- If you are seeking ADHD treatment without prior evaluation or prior treatment history, we advise completing a psychological evaluation first.
Duration and Nature of Treatment
- Psychiatric and substance use treatment typically involves regularly scheduled bi-weekly or monthly appointments. Stable medication management visits may be spaced one to three months apart.
- Patients are expected to maintain at least one annual physical exam and blood work twice a year and coordinate those visits with their primary care physician.
- If you stop scheduling follow-up visits for 120 days or establish a pattern of missed appointments, you may be considered discharged from the practice.
Prescription Refills
- Medication changes and refills are handled during regular appointments during business hours.
- We do not accept pharmacy refill requests for controlled medications and we do not replace lost or stolen controlled prescriptions.
- If a refill or new prescription is handled outside of an appointment, a $25 service fee may apply.
Medical Records
- Medical records may be requested and sent if approved.
- Fees are compliant with Florida statutes: $1 per page for the first 25 pages and $0.25 per page thereafter.
- The patient is responsible for any records fees not paid by the requesting party within 60 days.
Drug Screen Policy
- Your physician may order drug screening at any time if it is clinically necessary, including upon admission and randomly during follow-up visits.
- If you refuse required testing, you may not be seen and prescriptions may not be renewed.
- In-office drug tests have a $25 fee. Confirmation testing through an outside certified lab may create additional patient responsibility if insurance does not cover it.
Telepsychiatry Policy
- Telepsychiatry is provided through a HIPAA-compliant platform with audio and video capabilities.
- New patients may be offered an in-person or telehealth initial visit. Follow-ups are typically telehealth.
- You must be a Florida resident and physically present in Florida at the time of your appointment.
- You are responsible for having reliable equipment, a private location, and updated contact information.
- Your provider may require in-person monitoring, lab work, or drug screening when needed.
