Self-Pay and Office Fees

All major credit cards are accepted for payment.

Self-pay rates

  • Initial Evaluation (first session): $300
  • 30 Minute Follow-Up: $175

Fees not covered by insurance

Appointments must be confirmed at least 24 hours in advance. Unconfirmed appointments are subject to cancellation.

Late cancellations or no-show fees are automatically applied to the card on file, and missed appointment fees must be paid before a new appointment can be scheduled.

  • Initial appointment no-show fee: $200
  • Follow-up appointment no-show fee: $100
  • Late arrival up to 15 minutes, then no-show fee applies

Other miscellaneous fees

  • Medication refills outside of an appointment: $25
  • Letters to employers, schools, lawyers, and similar requests: provider hourly rate ($250/hour)
  • Comprehensive chart reviews: provider hourly rate ($250/hour)
  • Consultation with schools, insurance plans, and similar requests: provider hourly rate ($250/hour)
  • Medical records: $1 per page for the first 25 pages, then $0.25 per page thereafter
  • Drug tests: $25
